To find detailed information about a specific facility or types of facilities throughout the State of Georgia, use our Find a Facility search tool.
If facility inspection reports are available online, facilities are displayed in green. If there are state, other agency inspection or accreditation reports available online, you will see a “YES” under the column headed “Inspection Reports Available.”

NOTICE: You may experience problems opening our Ordering Information page or in trying to view survey reports. If this happens, you may have a firewall that your computer is using that is denying access to us. If it's the Ordering Information page, you must set your firewall to allow access to Port 443. In addition, you must set your browser to accept cookies, at least temporarily. If you can't open a survey report, you must set your firewall to allow access to Port 7001. Please refer to your firewall software manual for instructions or, if you are a business, and have an IT Department, call them with this information.
